Abstract Background Decompressive craniectomy (DC) is a frequently performed procedure to treat intracranial hypertension following traumatic brain injury (TBI) and stroke. DC salvage that reduces mortality at the expense of severe disability compromises quality life. The not without serious complications. Methods We describe complications its management in case-based review this article. Results Complications after are classified as early or late based on time occurrence. Early complication includes hemorrhage, external cerebral herniation, wound complications, CSF leak/fistula, seizures/epilepsy. Contusion expansion, new contralateral epidural, subdural hematoma immediate postoperative period mandate surgical intervention. It necessary repeat non-contrast CT head 24 hours 48 DC. Late hygroma, hydrocephalus, syndrome trephined, bone resorption, falls unprotected cranium.
